MRI ordering process
- For your reference and optionally to discuss with patients, review Sensible MRI's Patient History and Screening Form.
- When your patient arrives at our facility, our MRI technician will interview the patient and complete this form to ensure no pre-existing conditions exist that could adversely affect the procedure or the patient's well-being.
- Review and complete our MRI Order Form. If you need imaging order guidance from one of our radiologists, contact our office and we will provide contact information.
- Fax a copy to the number provided on the form. Give a copy to the patient to bring with them. Instructions and driving directions for the patient are found on the second page.
